Las Vegas testing: why one reading proves so little

Instability, not deficiency, is what makes perimenopausal bloodwork hard to read, and it is the main limit on hormone testing in Las Vegas. In perimenopause the problem is not that hormone levels are low. It is that they are unstable. Estradiol can spike above anything you saw in your thirties and then fall away within the same cycle.

A blood test captures one moment inside that swing. Two samples two weeks apart can put the same woman in two different categories, which is precisely why guidelines stopped recommending routine testing for women over 45.

Where testing does change decisions is in ruling things out. Thyroid function, ferritin and a full blood count between them explain a large share of the fatigue and low mood that gets attributed to menopause. There is more in fsh and estradiol testing, thyroid testing and understanding your results for the rest of it. Fatigue and low mood get attributed to menopause by default, and in Las Vegas a basic panel is often what corrects that assumption.

  1. When the picture is early

    Symptoms before 45 warrant investigation. A diagnosis of early menopause changes long-term bone and heart management, so it is worth confirming.

  2. When something does not fit

    Fatigue without flashes, weight change without cycle change, or symptoms that do not match the timeline. Thyroid and ferritin first.

  3. When there is no cycle to read

    After a hysterectomy, or on a hormonal coil, the usual marker is gone. Bloods carry more weight when the calendar cannot help.

  4. As a baseline before treatment

    Blood pressure, and bloods where your history warrants them, recorded before you start so the three-month review has something to compare against.

Answered the way a clinician would answer them, not the way a brochure would.

No. Menopause is a clinical diagnosis made from your age, your cycle history and your symptoms. Guidelines are explicit that women over 45 with typical symptoms do not need blood tests to confirm it. Testing has a role, but confirming the obvious is not it.
Three reasons. To rule out conditions that mimic menopause, chiefly thyroid disease and iron deficiency. To investigate an unusually early picture, where a diagnosis changes long-term management. And to set a baseline before starting treatment where that is useful.
Because FSH swings enormously from day to day while you are still cycling. A reading in the menopausal range on Tuesday can be entirely normal on Friday. One result tells you what your hormones were doing that morning, not what stage you are in.
It is usually worth it. An underactive thyroid produces fatigue, weight change, low mood, poor concentration and cold intolerance, and it is common in women in their forties and fifties. It is a simple test and finding it changes the treatment entirely.
Saliva testing is not validated for guiding hormone therapy and no professional body recommends it. Home finger-prick kits vary widely in quality and none of them can tell you where you are in the transition, because the underlying problem is that levels fluctuate rather than that nobody measured them.
